Upcoming Fee Change for Missed Appointments
Dear Valued Patient,
We want to inform you of an upcoming change to our missed appointment fee. Effective July 1st, the fee for missed appointments will increase from $35 to $50. We appreciate your understanding and cooperation in helping us maintain our schedule and serve all our patients effectively.
Appointment Cancellations/Reschedules/No-Shows Policy
Our business days are Monday through Thursday.
Your appointment date and time are reserved exclusively for you. If you need to cancel or reschedule an appointment, please provide us with advance notice of at least one full business day based on our business hours.
Cancellations or reschedules made with less than one full business day’s notice, or appointment “no-shows” will incur a $50 fee.
We will be unable to schedule any new appointments until all fees are paid. A credit card kept on file may be required for future appointments.

We understand that your time is valuable. We are dedicated to offering you superior patient care and streamlining the office visit experience. Our modernized scheduling system is designed to reduce paperwork and provide timely appointments for urgent skin issues.
With our direct pay model, patients pay for their visit at the time of service. We’ve structured our fee schedule with affordability and convenience in mind. We’ve found that with a large number of our clients, it is often less expensive to be seen in our office out-of-network than it is to be seen by an in-network provider. We do not require sign-up fees or membership fees, which makes our model different from a concierge practice that requires those fees. We’re committed to simplifying the paperwork and payment process, so you have more time to focus on receiving the care you deserve.
SUBMIT YOUR INSURANCE CLAIM WITH EASE
We provide everything you need to submit your claim to your insurance company so that they reimburse you for your visit. If you have a high deductible or high co-pay insurance plan, our direct pay model often will result in less out-of-pocket expenditure. Most private insurances have out-of-network benefits, so we will be able to provide you with a statement of service that you can submit to your insurance plan for reimbursement. At this time, If you have Medicare, you cannot be reimbursed for any of our services.

HealthShares offer health-conscious individuals and families an affordable way to share medical care expenses in a like-minded community. HealthShares like Zion Health and Liberty Healthshare are another option for patients looking for reimbursement for medical care from Dr. Kaplan and his team.
SCHEDULING YOUR APPOINTMENT
Appointments are scheduled to provide adequate time to discuss your health concerns. Office visit charges are based on the time spent with your provider. Our goal is to provide you with individualized, expert care during every second of your visit. If your visit requires additional procedures such as a biopsy and/or freezing, there will be a small additional charge. These additional charges will be discussed prior to doing the procedure. Any outside testing such as laboratory, x-ray, or pathology will be sent to the facility of your choice.
We are dedicated to offering transparent pricing during your visit. You can view rates and additional office visit charges in the office.
The following will be billed separately through your insurance to the laboratory or facility that you designate: Pathology, Laboratory, X-ray.
Cash, checks, and all major credit cards are accepted.
